| 中文摘要 | With the changes of human living environment and disease spectrum, cancer has become the leading cause of death and a major public health problem in China. Cancer is difficult to treat, has a long course of disease, a long treatment cycle and so on characteristics, need long-term companion and social support. Patients gradually lose their health and suffer from the pain and discomfort caused by cancer and treatment. At the same time, they are under great physical and mental pressure of survival and death, and suffer from mental health impairment. Spiritual well-being usually refers to a sense of meaning or purpose in life, inner peace and harmony, and the strength and comfort drawn from faith. Spiritual health can help cancer patient adapt to illness and is a source of strength that helps them cope with the pain associated with their illness. There are few domestic studies.This study visually analyzed and summarized relevant studies on spiritual health measurement in foreign countries (especially in cancer patients), understood the current situation in the field of spiritual health measurement, and analyzed the spiritual health status of cancer patients. To explore new ways to improve the quality of life of cancer patients.Research i. visual analysis of the research progress of spirituality measurement abroadCiteSpace V was used to visually analyze the current situation of spirituality measurement in foreign countries, and the leading authors, institutions and their cooperation were analyzed through the network node type. Through the analysis of keyword frequency, content analysis of research hotspots; The process of knowledge evolution was studied through the analysis of key words common current area view and abrupt node. The number of citations was analyzed through citation space. The results were as follows :(1) the number of articles related to spiritual health measurement increased year by year; (2) duke university and Koenig HG are the most published institutions and authors respectively; The latter works closely with Pargament KI and Exline JJ. (3) the keywords quality of life, validation, cancer, religion, health, depression were high centricity and high frequency, indicating that they were research hotspots. (4) time zone view indicates that the first research focus was on the spiritual health of patients with pain, and then the research content gradually materialized to focus on the spiritual health of cancer patients, end-stage patients and nursing staff; From 2013 to 2017, more new scales were widely used, such as FACIT-Sp (functional treatment of chronic diseases - spiritual status scale). "Quality of life" is the only purple circle node, which appears in 417 spiritual measurement literatures, indicating that spiritual health is closely related to quality of life.Study ii. Investigation on the spiritual health of cancer patients149 tumor patients were investigated by using the chronic disease treatment function assessment scale, including the quality of life scale, the FACIT-Sp subscale, the spirituality needs scale, and the life satisfaction scale. The results are as follows :(1) The quality of life, spiritual health and spiritual needs are all in the middle and upper level, and life satisfaction is in the middle and lower level. According to the analysis of each dimension, the social and family dimensions of quality of life score the highest, and the emotional dimension score the lowest. The score of significance dimension was the highest and the score of belief dimension was the lowest. (2) Demographic variables have an impact on life quality, spiritual health, spiritual needs and life satisfaction; (3) Pairwise positive correlation between quality of life, spiritual health, spiritual needs and life satisfaction; Each dimension of spiritual health is positively correlated with the quality of life, and the relevant level meaning is > and > belief. In all dimensions of spiritual needs, except for the dimension of supernatural relations, the other dimensions have a high correlation with life. (4) There is a mediating effect of all dimensions of spiritual health on quality of life. The significance and peace dimensions of spiritual health have a completely mediating effect on belief and quality of life. There is a chain mediating effect of belief ->, peace ->, meaning -> quality of life. (5) Spiritual health and quality of life play a complete mediating role on spiritual needs and life satisfaction. There exists a chain mediating effect of spiritual needs -- > quality of life -- > spiritual health -- > life satisfaction.Through the above analysis, conclusions and implications can be drawn: There are more and more studies on spiritual health in Europe and America, among which the quality of life is the main concern. Spiritual health and spiritual needs have an impact on the quality of life and life satisfaction of cancer patients, and belief in spiritual health plays a more important role in the quality of life of cancer patients through the indirect influence of meaning and peace. The way to improve the life satisfaction of tumor patients is spiritual need -- > quality of life -- > spiritual health -- > life satisfaction. Tumor patients have higher needs in the aspects of meaning and purpose, love and connection. Therefore, tumor patients should be made aware of their spiritual needs by various means, so as to meet their spiritual needs and improve their life quality and spiritual health. |
| 英文摘要 | 癌症患者需要长时期陪护和充分社会支持。这种慢性疾病病程长、治疗周期长并且大多难治。随着人类生活环境的变化，疾病谱也不断改变，癌症逐渐成为中国人的首要死亡原因和主要公共健康问题。癌症关乎生死，患者承受巨大压力，在逐渐失去健康的同时还承受疾病治疗所带来的多重痛苦，其（灵性）健康状态严重影响。Peterman提出，灵性健康指意义感或生命目的、内心平安和谐，以及信念之力与慰籍。灵性健康能帮助患者适应其疾病，是其应对疾病相关痛苦的力量来源。目前，国内灵性健康相关研究却很少。本研究可视化分析并综述国外灵性健康测量（特别是肿瘤患者灵性健康）相关研究，了解目前灵性健康测量领域现状，分析肿瘤患者灵性健康状况；以探索有效提升癌症患者生活质量、生命质量的新方法。研究一、国外灵性测量研究进展的可视化分析采用CiteSpace V可视化分析国外灵性测量现状，通过网络节点类型分析该领域领衔作者、机构及其合作情况；通过关键词词频及内容分析研究热点；通过关键词突发性节点及共现时区视图分析研究知识演进过程；通过引文空间分析引文数量。结果如下：（1）灵性健康测量相关文献发文量逐年增加；（2）杜克大学和Koenig HG分别是发文最多的机构与作者；后者与Pargament KI、Exline JJ合作密切。（3）关键词quality of life、validation、cancer、religion、health、depression为高频及高中心度关键词，表明其为研究热点。（4）时区视图提示最早研究热点关注疼痛患者灵性健康，之后研究内容逐渐具体化，关注癌症患者、终末期患者、护理人员的灵性健康状况；2013年到2017年更多新量表得到广泛应用，如“FACIT-Sp（慢性疾病功能治疗-灵性状况量表）”。“quality of life”是唯一紫色圆圈节点，出现于417篇灵性测量文献中，表明灵性健康与生活质量密切关联。研究二、肿瘤患者灵性健康相关调查应用慢性病治疗功能评估量表包括生活质量量表FACT-G 及灵性子量表FACIT-Sp、灵性需求量表、生活满意度量表对149名肿瘤患者进行调查。得出如下结果：（1）生活质量、灵性健康、灵性需求均在中上等水平，生活满意度处于中等偏下水平。具体各维度分析，生活质量各维度社会和家庭维度得分最高，情感维度得分最低；灵性健康各维度意义维度得分最高，信念维度得分最低；（2）人口学变量对生活质量、灵性健康、灵性需求、生活满意度有影响；（3）生活质量、灵性健康、灵性需求、生活满意度之间两两正相关；灵性健康各维度与生活质量呈正相关，相关水平意义＞平和＞信念。灵性需求各维度中除与超自然关系维度，其他各维度均与生活有较高相关性；（4）灵性健康各维度对生活质量存在中介效应，灵性健康中意义及和平维度对信念和生活质量的起完全中介作用，存在信念 ->平和 ->意义 ->生活质量的链式中介效应；（5）灵性健康和生活质量对灵性需求和生活满意度起完全中介作用，存在灵性需求 ->生活质量 ->灵性健康 ->生活满意度的链式中介效应。通过上述分析，可以得到结论和启示：欧美灵性健康研究越来越多，其中生活质量是主要关注点。灵性健康和灵性需求对肿瘤患者生活质量和生活满意度有影响，灵性健康中信念通过意义和和平的间接影响在肿瘤患者生活质量中发挥着更重要的作用。提高肿瘤患者生活满意度的路径为灵性需求 ->生活质量 ->灵性健康 ->生活满意度，肿瘤患者在意义与目的、爱与联系方面有更高的需求。因此应通过各种手段使肿瘤患者意识到其灵性需求，进而满足其灵性需求，通过提高生活质量，灵性健康水平达到提高生活满意度的目的。 |
