| 中文摘要 | 目的探讨~(99)Tc~m-4,9-二氮-3,3,10,10-四甲基十二烷-2,11-二酮肟(HL91)判断脑胶质瘤恶性程度的价值及其与葡萄糖转运蛋白1型(Glutl)表达的关系。方法 24例脑胶质瘤患者,注射~(99)Tc~m-HL91 10 min、2及4 h 后进行头颅前后位、后前位和左右侧位静态显像。在平面图像上,利用感兴趣区技术,分别计算不同时相肿瘤与对侧正常脑组织的放射性比值(T/N)。所有患者1周内均行手术治疗,将切除标本做 HE 染色、免疫组织化学检查。结果 HE 染色证实,24例患者中高度恶性17例,低度恶性7例。所有高度恶性和2例低度恶性患者~(99)Tc~m-HL91显像阳性,其余5例低度恶性患者为阴性。高度恶性组和低度恶性组10 min 及2、4 h 的 T/N 比值分别为1.775±0.386、1.861±0.388、1.881±0.434和1.067±0.125、1.140±0.009、1.110±0.139,组内三时相间 T/N 比值差异无显著性(P>0.05);低度恶性和高度恶性肿瘤 T/N 比值间差异有显著性(P<0.001)。17例高度恶性和3例低度恶性患者 Glutl 表达阳性。且 Glutl 的表达在高度恶性胶质瘤患者[(41.91±20.53)%]明显高于低度恶性患者[(8.88±13.85)%]。胶质瘤对~(99)Tc~m-HL91的浓聚与 Glutl 的表达呈明显相关性(r=0.888～0.964,平均 r=0.919,P<0.001)。结论 ~(99)Tc~m-HL91在高度恶性脑胶质瘤中有较高浓聚而在低度恶性胶质瘤中浓聚较少,可用于鉴别脑胶质瘤恶性程度;~(99)Tc~m-HL91在胶质瘤中的浓聚与 Glutl 的表达呈明显相关性。; Objective To evaluate the relationship between ~(99)Tc~m-4,9-diaza-3,3,10,10-tetrameth- yldodecan-2,11-dione dioxime(HL91)imaging of malignancy of brain gliomas and Glutl expression. Methods Planar brain SPECT imaging was performed in 24 brain gliomas patients at 10 min,2 and 4 h af- ter injection of ~(99)Tc~m-HL91.Regions of interest(ROIs)were drawn,and the radioactivity ratios of lesion- to-normal tissue(T/N)were calculated.All patients were operated within a week and the resected samples were stained with hematoxylin and eosin(HE)and immunohistochemistry.Results ~(99)Tc~m-HL91 was highly uptaked in all 17 high malignant gliomas with positive HE staining and also in 2 of 7 low grade gliomas.T/N ratio of the high malignant and the low grade gliomas at 10 min,2 and 4 h were 1.775±0.386,1.861± 0.388,1.881±0.434 and 1.067±0.125,1.140±0.009,1.110±0.139,respectively,showed no sig- nificant difference(P>0.05)between them in each group.However,there was significant difference be- tween the low grade and the high malignant gliomas groups(P<0.001).Besides,Glutl was expressed in all the high malignant gliomas and 3 of the low grade gliomas.And it was significantly higher in former group[(41.91±20.53)%]than in latter group[(8.88±13.85)%].Uptake of ~(99)Tc~m-HL91 and expres- sin of Glutl in the high malignant gliomas were strongly correlated(r=0.888～0.964,mean r=0.919,P< 0.001).Conclusion ~(99)Tc~m-HL91 had a selective concentration in high malignancy brain gliomas and its uptake and expression of Glutl were highly correlated.; 2002年度河南省科技攻关项目(223034400) |
