| 中文摘要 | 目的观察超声引导瘤内注射90钇(90Y)—高温碘油悬液治疗肝癌的疗效。方法在超声引导下对治疗组12例肝癌患者的27个结节行90Y—高温碘油悬液瘤内注射治疗,对照组10例肝癌患者的23个结节行90Y—普通碘油悬液瘤内注射治疗,两组每个瘤灶内均给予1ml/cm3剂量的碘油与1.0mCi/cm3剂量的90Y悬液。结果除肿瘤外,两组患者其他脏器内均未发现90Y沉积,未见有心、肺、肝、脾、肾及骨髓的异常改变。不同时间点肿瘤均呈缩小趋势,治疗后30d时治疗组肿瘤生长率为-0.47±0.17,显著低于对照组(-0.18±0.22),P<0.01;对照组肿瘤坏死范围均未超过40%;治疗组肿瘤坏死率在3、7、30d时分别为72%、81%、87%,与对照组比较,均有显著性差异(P<0.01);各时间点治疗组坏死率呈增加趋势,差异显著(P<0.05)。结论超声引导瘤内注射90Y—高温碘油悬液治疗肝癌疗效确切。; Objective: To observe the effect of hot lipiodol with~ 90Y intratumor injection for treatment of liver cancer. Methods: 12 patients in treatment group and 10 patients in control group were performed ultrasonically guided hot lipiodol injections (treatment group) and common lipiodol injections(control group)with radiopharm-aceutical Yttrium-90 therapy. Results: No side-effect and no significant complication were observed. The tumor sizes showed a reducing trend at different time. In treatment group,the tumor growth was-0.47�17 at the 30th day after treatment, and significantly lower than that in control grout(-0.18�22) (P<0.01).The tumor necrosis range was less than 40% in control group. The tumor necrosis rate in treatnent group was 72%,81% and 87% at the 3th,7th and 30th day respective. There was a significant difference between the two groups(P<0.01). There was a positive correlation between the necrosis rate and the time in treatment group (P<0.05) Conclusion: Intratumor injection of hot lipiodol with~ 90 Y is a safe, feasible and effective method to treat liver cancer. |
